The FEMA Housing Inspection Services Program provides emergency grants, often within hours of inspection, to aid survivors whose homes have been affected by presidentially declared emergencies or natural disasters in the United States. Vanguard Emergency Management has been contracted by FEMA to respond to and coordinate home inspection activities for Individual Assistance disasters.

Forms and Documents

The following forms need to be downloaded and returned to Vanguard. Proper completion of these forms is required for payment and expense reimbursements! 

Either bring the completed forms with you to your training, or mail them directly to the Vanguard office at 188 Brooke Rd, Suite 300 Winchester, VA 22603.

Electronic Expense Reporting Guide Guidance for completing an electronic expense report through Risk Manager
Expense Report Template – Fingerprinting/ Badging/ Adjudication This form is for use by independent contractors who are travelling at the request of Vanguard in order to get FEMA security clearance.
Vanguard Travel Policy - Overview Use this form for guidance on what travel expenses are allowable and reimbursable.
Expense Summary Document Use this form for reimbursement of travel expenses related to your training. Instructions are included in the form.

When this form is complete, and you have attached the necessary receipts/ backup documentation, please forward the entire package to:

Note: expense reimbursements cannot be processed until the following documents are completed and returned to Vanguard

W9 Form

Both of these forms contain information essential to Vanguard’s ability to pay its inspectors.  We will not be able to pay you or reimburse your expenses unless these form are completed and returned. Please note that if you do not SIGN the W-9 form it cannot be processed.

Selecting direct deposit ensures the fastest possible payment.

The information provided will be kept private and destroyed according to company policy.
Independent Contractor Agreement Review the entire document then sign and date page 6.

Only Page 6 needs to be returned, but please keep a copy of the full contract for your records.
Small Business Reporting Form This form is required for Vanguard’s federal reporting obligations.  Please complete sections 1 and 2 (required) and section 3, if applicable.

If certification information is available, please complete section 4.

Note: that independent contractors qualify as a “Small Business Concern” even if not certified.
